Fees and Forms

For every new client, we begin with an initial diagnostic intake appointment.

Prior to your first appointment, we will send you a link to register as a new client and complete intake forms through our  Electronic Health Record (EHR), Therapy Appointment.

We welcome a number of insurance plans.

We are in-network providers for the following behavioral health plans.



In addition, a number of other insurance companies offer out-of-network benefits that can be used to meet with our counselors. If your insurance network is not listed, you may contact your insurance company to inquire whether your plan offers out of network benefits.  Whenever applicable, we can submit claims to your insurance company for use of your out of network benefits.

 We also offer appointments based on a sliding fee.  We offer a discount of up to $70 per session for our sliding fee arrangement.  Please  contact us for more details about sliding fee discounts.

Payment Options

Ervin Counseling accepts cash, check, or credit card. Please make checks payable to Ervin Counseling . Payment is due directly after services are rendered, unless otherwise noted by insurance company policies. Payment plans are also available. If you wish to discuss a written agreement that specifies an alternative payment arrangement, please ask us.

When you register as a new client you will be asked to enter a credit card into our encrypted system.
