For every new client, we begin with an initial diagnostic intake appointment.

Prior to your first appointment, please complete the Patient Registration and Biographical Information web-forms at TherapyAppointment.com.

Please read these instructions for how to access and complete these forms. 
Please also download and carefully read the Disclosure Statement & Agreement for Services. We will need a signed copy for your file.

In addition, a number of other insurance companies offer out-of-network benefits that can be used to meet with our counselors. We are happy to help you work with your insurance company to get quotes for counseling benefits. In some cases, we can submit claims to your insurance company on your behalf.

Payment options

Ervin Counseling accepts cash, check, or credit card. Please make checks payable to Ervin Counseling and Family Life Ministries. Payment is due directly after services are rendered, unless otherwise noted by insurance company policies.
Payment plans are also available. If you wish to discuss a written agreement that specifies an alternative payment arrangement, please ask us.
